  Creating or Revising an EICCD CDM
 
  1.  Introduction:
  A CDM is created by a faculty member when a new course is proposed. A CDM is revised during the General Education Review Process or when a faculty member believes that more than twenty percent of the CDM needs to be updated. This sheet explains how to create or revise a CDM. The CDM Approval Process Instructions provide information on how to move the new CDM through the District for final approval by the Instructional Council.
 
 
  2.  Introduction: Finding the Form
  The CDM form has a cover sheet attached and is designed for electronic sharing and storage. The blank form is available on the P drive in the folder for Institutional Research and on the Webpage under Institutional Research. It is a form that you must copy to your computer to use.
 
 
  3.  Information Needed to Complete the Form:
 
  Here are descriptions of the information
that is required in each of the fields of the form:
 
  EICCD COURSE DEVELOPMENT MODEL
 
  CATALOG COURSE NUMBER: Put the assigned catalog number here (also called alphanumeric).
              For Example, BI:125
 
  COURSE TITLE: Use a maximum of thirty letters for the title here.
 
  Semester Hours Credit: Put the numerals here for the number of credit hours proposed for the course. One credit hour is equal to 16.5 hours of class clock time. On this form, indicate how many hours per week the class will meet.
 
Lecture Hours   Lab Hours   Other Specify:
  (e.g., Coop, Clinical, etc.)
Per Week:   Per Week: (if applicable)   Hours Per Week: (if applicable)
 
  Course or courses this CDM replaces, if any: This information may be used by the registrars.
 
  CATALOG COURSE DESCRIPTION: Put the description proposed for the college catalog here. Use primarily phrases, not all complete sentences. Articulations with other colleges and universities use the catalog descriptions.
 
  ENTRY LEVEL SKILLS/KNOWLEDGE:
 
  Recommended: If there is a recommended level of basic or technical skills (reading, writing, math, computing, etc.) needed before students enroll in the course, list them here. Use this suggested language, please: Ability to register for [EN105] as indicated by college assessment. Please know that this recommendation will not appear in the catalog or class schedule.
 
  Required: If there is a validated level of basic or technical skills (reading, writing, math, computing, etc.), list them here. Use this suggested language: Knowledge equal to [MA:096] or [two years of high school algebra] and minimum [math] placement scores. This comment is listed in the catalog and schedule.
 
  PREREQUISITE COURSES: If there is a prerequisite course or courses, list them here by catalog number. These are courses that the student must have successfully completed in order to be successful in this course. Prerequisites courses must be agreed upon by the faculty and approved by Instructional Council. These will be listed in the catalog and schedule.
 
  CO-REQUISITE COURSES: List any courses that must be taken at the same time here. Co-requisite courses must be agreed upon by the faculty and approved by Instructional Council. These will be listed in the catalog and schedule.
 
  TEXT BOOK(S) USED FOR THE CDM DEVELOPMENT: List the title, author, and publication information of any texts used as the basis for developing this CDM. This does not mean the text will be used by everyone teaching the course.
 
  GENERAL COURSE GOALS: These are two or three broad goals stating the general outcomes of the course; these will be statements about what a learner should be able to know and do once he or she has met of all the requirements of the course. These should reflect the Graduation Requirement General Education Discipline-specific Goals for EICCD. The statements should describe what is to be learned, what level of learning is to be achieved and in what context the learning is to take place. (See the EICCD handout on writing course goals for guidance.)
 
  COURSE COMPETENCIES/OUTCOMES: In this section list a more specific set of learning outcomes for the topics covered in the course. These statements should include knowledge, skill and value outcomes as are appropriate for the course curriculum. The statements should describe what is to be learned, what level of learning is to be achieved and in what context the learning is to take place. (See the EICCD handout on writing course goals for guidance.)
 
  METHODS OF INSTRUCTION MAY INCLUDE: List here the various methods that are likely to be used. This may include: lecture, demonstration, lab, supervised practice, simulations, virtual experiments, group activities, discussion, etc.
 
  EVALUATION METHODS MAY INCLUDE: List here the methods that are likely to be used to assess student mastery of the course learning outcomes, such as written exams, oral presentations, homework, long or short research reports and papers, group work, etc.
 
  ATTENDANCE: Always use this language here: Policies on attendance will be formulated by the instructor and communicated to the students on the course syllabus.
 
  ACADEMIC DISHONESTY: Always use this language here: Policies on academic dishonesty can be found in the EICCD student code of conduct published in the student handbook. Individual faculty member's policies concerning academic dishonesty should be communicated to students in the course syllabus.
 
  TOPICAL OUTLINE: Enter an outline of topics or unit headings for the courses here. The order may be the one used in the course, or it may be addressed in a different order.
 
  CDM AUTHOR INFORMATION:
  Written By: List here the faculty members' names who wrote the original
                     CDM for this course.
  Date Written: Enter here the date the original CDM was created.
  Date Revised: Enter here each date the CDM is revised.
  Revised By: List here the faculty members who revised the CDM.
  Date Reviewed, no changes needed: Enter here the date the CDM
                                                              is reviewed, but not changed.
  Reviewed By: List here the faculty members who reviewed the CDM.
