Application Form

Dear Pre-Nursing Student:

To assist us in the selection of our Fall 2013 Nursing Fundamentals class, please complete the information sheet.

The deadline to complete and return the form is Sunday January 27, 2013. YOU MUST COMPLETE AND RETURN THE APPLICATION.   APPLICATIONS RECEIVED AFTER THIS DATE WILL NOT BE CONSIDERED FOR THE FALL 2013 SEMESTER.

Those who have met the following criteria will be given priority admission into the Nursing Program:

1. The student must have completed high school or obtained a GED and the transcript must be on file at a campus within the Eastern Iowa Community College District (EICCD).
   
   
2. A cumulative G.P.A. of 2.0 or above is required on all co-requisite courses for admission into the nursing program.  The Student must have achieved a “C” or better in all required courses.  This is also a graduation requirement for Nursing.
   
   
3. Priority Admission will be given to students who have completed the most required courses with a “C” or better grade:
   
   
   • *Anatomy & Physiology I        
   • *Anatomy & Physiology II
   • *Human Growth and Development  
   • *Nutrition General Psychology I
   • General Psychology I
   • English Composition I
     
     *Note: Must have been taken within 5 years of enrolling in Nursing Fundamentals
     
     
4. Official college transcripts for transfer of credit must be evaluated prior to being considered for acceptance into the Nursing Program.
   
   

Thank you for your interest in the EICCD’s Nursing Program.  Please do not hesitate to call me if you have any questions regarding our Nursing Program admission procedures. Students accepted into our Fall 2013 Nursing Program will be notified the week of March 11, 2013.
