ADDITIONAL FEES: Iowa Exams
State written test fee is $45 and state skills test fee is $75. These fees are not included in the cost of the class. Subject to Change

REGISTRATION

Our conversion to a new computer system will make it easier for you to register. In order to make the enrollment process as smooth as possible, we ask that you make your payment at the time of registration.

Mail - Include credit card information for Visa, MasterCard, American Express or Discover or enclose a check payable to Eastern Iowa Community College District (please put your name, address, company billing information, phone number, social security number and five-digit section number on the check), and mail it to Eastern Iowa Community College District, Attn: Registration, 306 W. River Drive, Davenport, IA 52801.

Information we need - However you choose to register, we need the following information:

1. Class name and five digit section number
2. Your social security number
3. Your name, address, phone number
4. Payment information - please provide us with your credit card number or company billing information

Approximately one week before the class starts, you will receive a calendar in the mail with the dates and times of your class. You will also receive a medical history form. The top part of the form is to be filled out by you, and the bottom part is to be taken to your doctor for your TB www.

Refunds and Substitutions - If your plans change, you may cancel your registration up to seven calendar days prior to the start of the program. After that, no refunds will be issued.

You may however, apply the tuition towards another class that meets your scheduling requirements or you may choose a gift certificate in the amount of your class to use within one year of the date issued. Please note that if you don't cancel and don't attend, you are still responsible for payment.
